Purpose of these Forms

This form may be filled out by the alleged victim/survivor (complainant) of conduct prohibited by Rule 1.6, mandatory reporters as outlined Rule 1.6 (including WVU faculty, staff and/or employees), or any other person who witnesses or has knowledge of such alleged conduct.

If you wish to remain Anonymous, you may seek services via our 24/7 Equity Assurance on-call phone at (304) 906-9930 or file a complaint with this request. However, please note that the ability to investigate an anonymous complaint is severely limited.

For the confidentiality of all parties, please understand that we may not follow up with you unless you are directly involved in this case, but we will always follow up with the people who were directly affected by the behavior you described in this complaint form. For example, we would not typically reach out to a professor who filed a complaint about something they heard secondhand from a student unless we have follow-up questions. Instead, we would contact the student directly to make sure they were OK. We hope you understand that we hold confidentiality in the highest regard.
